Home State Health and the Centene Foundation have announced a new $750,000 grant initiative aimed at improving health outcomes across Missouri by supporting community-based organizations and healthcare providers addressing social drivers of health.
The program will distribute funding through competitive grants ranging from $10,000 to $100,000, with a total of 20 awards expected. Applications are open through July 10, 2026, and selected recipients will be announced in August 2026.
The initiative focuses on strengthening community programs that address key factors influencing health, including food insecurity, housing stability, workforce development, and support for rural communities. Eligible applicants must be Missouri-based nonprofit organizations serving Medicaid populations or communities with significant Medicaid-eligible residents.
According to Home State Health leadership, improving health outcomes requires addressing the root social and economic conditions that affect wellbeing. The organization emphasized that access to stable housing, nutritious food, and employment opportunities plays a critical role in reducing barriers to better health across the state.
Funding will be distributed across multiple tiers, including grants of $10,000, $25,000, $50,000, $75,000, and $100,000, allowing a range of organizations to benefit depending on program size and scope.
The Centene Foundation, the philanthropic arm of Centene Corporation, supports initiatives that aim to improve health equity and expand access to essential services. The foundation’s broader mission focuses on addressing healthcare access, education, and social services in underserved communities.
